Can you put tempered glass on the iPhone Duo's folding screen?

Not on the inner display. The 7.6-inch panel folds, so it needs a flexible film that bends with it — rigid tempered glass would crack or force the hinge. The 5.4-inch outer cover display does not fold, so it can take glass protection. That is why iPhone Duo protection is sold as a matched pair rather than a single sheet.

Will a screen protector make the crease worse or peel at the fold?

A protector made for the Duo will not. It is cut to the folding area and uses an adhesive designed to flex through tens of thousands of open-and-close cycles rather than lift at the hinge.
